Page Size with Gutter

I am creating a US Trade 6x9" book with .5" margins. How do I set up the gutter for a 170+ page book? Do I add the gutter into the page size from 6x9 to 6.3x9 or do I change my inner margins so they include the gutter, but then my left and right margin are not equal.
