copyright page

So I understand I need a page after my title page as my copyright page. I plan to upload tomorrow. Do I just leave this blank? Do I try to write it in myself? I'm guessing when I fill in that portion as I prepare to upload it will auto populate that blank page. Is that right?
I found this from Lulu-

The page that typically appears after the title page, containing the artistic property protection (license). If the book has an ISBN then the ISBN must also be included on this page.

You, as the creator, are both the copyright holder and publisher of all works created and distributed using the tools and services available on the Lulu.com website. When creating your work's copyright page, you should list yourself as the copyright holder and publisher.

Example:

Copyright © 2013 YOUR NAME. All rights reserved.
Published by YOUR NAME / COMPANY HERE
ISBN 123-4-567891-01-2

Should I just copy this onto my blank page and change the information?
