my first .docx ebook file

I am uploading and converting my first book on lulu from a .docx document to epub usig the lulu converter.  I read the conversion guide, and followed it, but am still not getting my chapters to become a linked TOC in the left pane of the ereader.  I turned all the Chapter headings to heading 2 like the guide specifies so I thought it should work.  I am also getting a blank page in the beginning of my document for some unknown reason.  Since this is my first shot, and I want to learn to do this myself, could someone take a moment and have a look.  I'd apprecaite it. Whoever is willing to look, i guess I will have to send the .docx file.
