where do you send the W8-BEN form to?

What address should the W8-BEN form be sent to?

Do you need to add a covering letter or some sort of Lulu account ID to the form so they can identify whom it belongs to?

I'm from the UK but have an ITIN number.

I am also looking for the snail mail address to send the W8-BEN form.

mrasell - When submitting the form to other payers I have included my user name and email address on Line 8 of the form so they can match the form to my user account. I am assuming lulu will be the same.

I've been researching this and checking through old forum posts.  

Thumbs down, Lulu.com, for not providing a clear mailing address to the correct Lulu finance department on the same page as author remittance settings. Every other place I publish at provides clear instructions: Smashwords, Amazon.

It's form W-8BEN, not W8-BEN. The other forum post is helpful.

But Lulu should have this on the remittance settings page or somewhere in the accounts page as an easily clickable link to the necessary info.

Thanks frontain I appreciate your help and I agree it would be more useful to have this information as part of the accounts page or the like.
