Where at Lulu do I submit a completed W-9?

I have recently published two books through Lulu. I have received some royalty revenue and I need to submit a completed W-9 form. Where do I do so?

This has the address, but be warned: this is an old thread from the previous platform; the second link I posted here has the most recent info I could find on where to submit your W-9.
